Text Box and Form Fields

In Word 2003, I created several text boxes that automatically located
themselves on a drawing canvas. I would now like to make the text boxes form
elements, however the form toolbar is grayed out whenever I try to put the
cursor in the text box.

The help file does not have a provision for this condition. Am I doing
something wrong? How do I make a text box a form field?

My overall goal is to create a template that allows people to enter text in
fixed size and location text boxes.
 
Delete the text boxes (and the drawing canvases) and replace them with
table cells to provide holders for the form fields. You can either use
single-cell tables that you drag into position, or make a single
larger table whose cells you size as necessary.
