do you mean a ordinary template or a DWT (dynamic web template)?

If a DWT see

This is for FrontPage 2003...

Which version do you have?

If you mean an ordinary template you make your template

then File | Save As in the 'Save as Type' box hit the down arrow and scroll
to the bottom for 'FrontPage Template (*.tem) and press Save your template
will be saved as a fp template.

If you just mean how to do you make a web page as a template, with a layout,
header, footer, menus...

You can use CSS to make div's and use also for your formatting.

Or you can use Tables, there are several choices of table layout's in
FrontPage which you can tweak if you wish or have a play and make your own,
along with includes for ease of use.

We need to know the version your working with and exactly what you want to
do of the above.
