Check the templates that are included with Access, and, if you don't find
one that will work for you, check further at http://office.microsoft.com. I
know there is one that keeps track of records, CDs, etc., which could be a
starting point, but would require some modification. That is the only source
of Access templates of which I am aware -- there's no business model to make
it attractive for anyone to go to the extra work to create a template, so
most people doing freebies just make available sample databases.

Two good places to start looking for samples, because they link many Access
websites, are http://www.mvps.org/access the FAQ site for the USENET
newsgroup comp.databases.ms-access and the Resources section of
http://ntaccess.tripod.com (a website I created for the Access SIG of my
user group, but which for technical reasons I am no longer able to update).
Using http://groups.google.com will allow you to search the archives of
most/all of the microsoft.public.access newsgroups and the USENET newsgroup
previously mentioned.
