If the template you want is not installed with Access or at the
http://office.microsoft.com "Office Online" site, it is unlikely that one
exists. There is no valid business model for Access templates, and for
those volunteering their assistance, it is much easier to construct a
sample.

I don't recall seeing posts about a sample database for the purpose you
mention, but the archives of these Microsoft-sponsored newsgroups, as well
as the USENET newsgroup comp.databases.ms-access can be found and searched
at http://groups.google.com.

And, you might get lucky, and someone might respond offering you either a
sample, or a copy of their database application for the purpose. Or, maybe
someone more familiar with the templates provided by Microsoft could suggest
one that might be adapted to your needs.
