template for credit card receipt

I need an Office template for a credit card receipt, but have not been able
to find one to download in the templates area. A simple 1 page credit card
receipt, or otherwise 3 to a page. I'm currently using Office 2003. Will
switch to 2007 but will be sure to click on the option of having it also read
Office 2003 documents.

Anyone?
 
If you cannot find something suitable at Google that you can modify, you'll
have to make your own.
