Hello.. I am about to set up a new business and incorporate it. The
corporation will have 100 shares.. I am looking for a template to set up a
contract offering 20% of the business for certain services needed to run the
business.. This person will be one business entity and I will be another
business entity.. and we will form a business together with a 20/80 ratio of
shares for this new business.. I hope this is making sense. Thank you cm

You're looking in the wrong place. Microsoft Access is a database program, and
the "forms" in this groups title are data editing screens in Access - not
paper business forms.

Microsoft almost surely does not have a template all set up (with specific
percentages yet...!!!) for your specific application; and business
incorporation laws differ from one jurisdiction to the next. Contact a local
attorney - you're probably not going to get this online, and if you do you may
not be legally covered anyway.
