Tasks and reminders show twice

Hauser Chief

Tasks and reminders show up twice and if I delete one both are deleted. How
can I fix this?
 
Before we can diagnose or prescribe assistance, you need to tell us some
more information. Start with:

version of Outlook
where exactly do you see 2 copies of each Task?
what add-ins do you have running?
did it work before and what happened since it did work?
