Table of Contents skips level 2 items

I have an outlined Word doc. When I create a TOC, the Level 2 letters "a)",
"d)", "e)", and "f)" are skipped. What can I do to correctly format these
Level 2 items so they will appear when I update the entire TOC?
 
Nevermind, I had to used the outlining tool bar, select outlined text,
increase the level from "body text" to "level 2"
 
Is the same heading style applied to these paragraphs as to those that do
appear? Are they perhaps in a text box?
