Table of contents is duplicating entries

I can't figure out why this is happening... a few of the entries on my table
of contents are showing up twice, and they're definitely not in my document
twice. It's only happening at the TOC1 level and TOC2, 3, etc headings are
only appearing after the second entry. Here's an example:

1. introduction
2. stuff
2. stuff
a. blah
b. blah 2
3. even more stuff
4. more and more
4. more and more
a. stuff
b. stuff 2
c. stuff 3
5. end

Any help would be greatly appreciated.

The usual cause of this is a page break before the heading (which has gotten
inside the bookmark for the heading). Instead of using manual page breaks,
try to get by with "Keep with next" and other tricks to control text flow.
If necessary, apply the "Page break before" property to the heading.
