Switchboard form no longer fills



I have a file that has been built over the years. This past month we updated
all computers to Microsoft 2007. Now when we open the file and go to the form
named "switchboard" the form comes up but there is only one command
button(Name: Option1) and it has no labels that are filling. The guy that
built this is long gone and I am left with just enough to hang my self.
I have added the code I found for this form. Please let me know what I can
do. Thanks in advance and let me know if I can give anymore direction as to
what I am looking at.
