I have received the below (blue screen) error
intermittently for the last year without resolve. I have
completely rebuilt this machine including all software,
driver and bios updates. I do have an HP d145 hooked up
to this machine and did find other threads that indicated
this could be an issue. Has anyone had this problem and
found a fix? Thanks!

Its a USB driver issue try and update the drivers of any devices
attached to the USB ports

I started down that road in the beginning. Prior to rebuilding the system I updated all USB device drivers first then I went through every device in or attached to the computer to look for driver/software updates. Nothing improved. Then I completely rebuilt the system and repeated the process without resolve. I have a feeling it is the HP d145 which is hooked up via USB but who knows? It is hard to troubleshoot because it is so intermittent, but when it does appear it can repeat for a few days. I will keep searching. Thanks

I got the same problem this morning and could not get my machine to run other than in safe mode. Found the answer here - http://h10025.www1.hp.com/ewfrf/wc/...en&cc=gb&lc=en&dlc=en&os=181&docname=c0009382

I solved the problem (not counting lots of reboots) by:
1. uninstalling the HP softwar
2. renaming the UHCD.SYS file to .ol
3. downloading SP4 for W2K and reinstalling SP
4. downloading the latest D145 driver and reinstallin

This did not work so I deleted the USB devices in Hardware Manager and had them recreate by using refresh hardware devices and hey presto it's fixed
My conclusion is that I have recently installed FrontPage 97 and it had banjaxed the USB drivers but reapplying SP4 updated the drivers and that sorted it
