[SPAM] in subject line

Everytime I recieve an email [SPAM] appears in the
subject line. I am using MS Outlook 2003
 
Outlook doesn't do this. What sort of add-ins are you running? What kind of
mail account?
 
It is probably inserted by your email server that has automatic Spam
detection. This would make it easy for you to build a filter for spam email
in outlook using rules.
