I am presently using K9 with OE6, but you still have to spend time waiting
for K9 to sort out the spam from the good email.
Is there a program which can block spam from ever leaving your mail box at
the ISP location?

It's been a while since I used it, but Mailwasher was of great help to me
once. It only downloads headers, and then it can filter according to your
personal settings. It can even look up every sender against public
blacklists, immediately removing 90% or more of your spam. Making custom
filters to get rid of stuff like high volume viriis is also possible, which
made this program indispensible to me when I was getting several megabytes
worth of Swen-mails per day.

Not sure what is the latest free version. Google or ask around.

You can use the Server Check function of K9 to view all your mail on
the ISP before it gets downloaded. Then you can choose to delete the
messages at the server level.
