Signup form

I am trying to create a signup form to use for a contest that I am
helping with. I would like the form to be run through PowerPoint, and
for the results to be stored in a text file or Excel spreadsheet. I
saw instructions a few years ago on how to do this, and even did it,
but my test copy of that file is now missing. I don't mind using VBA
at all, and it doesn't scare me. Can anyone help? Thanks!
