Setup for Symbols to Change when Activated

I have been trying to build a form that my supervisors can use with little
difficulity. I need to find out how you can insert a symbol (empty box) and
when you click (or double click) in the box it will change to having a
checkmark or an 'X' inside the box. Any help would be deeply appreciated.

I am using MS Office 2003 with Windows XP Pro
