Setting up a columnar report

Hello all,
I'm trying to set up a columnar report and have the the records display as
columns. The number of records can be unknown but not more than fifty.
I'm having trouble with 1) getting the data to show up, and 2) having the
labels print only once on the left side.
Can someone point me in the right direction here?
