Sending outlook forms

Hi - I 've created an outlook form, but when I email it, the receipient only
receives a empty/blank email. I wanted the form to do a customer survey
where the customer clicks on various parts and replys
 
What kind of emailo setup does the customer have? I think that only works if
the receipient has outlook with exchange server?
 
I'm having the very same issue...when I send the form, all they receive is a
blank page...hopefully someone will respond soon...AE

To e-mail a form, you need to insert it as an Item instead of a File. This
feature is on my toolbar. It looks like a paperclip. If you click the down
arrow, you will see ITEM. I hope this helps!
 
I had a similar issue, in the Form toolbar there are two options 'Edit
Compose Page' and 'Edit Read Page'. Once you have created the form in the
compose page, making sure that each field has individual properties set copy
and paste the fields from the 'Edit Compose Page' acrross to yous 'Edit Read
Page'.

If the fields have been set individually they should now be active to the
recipient.
