Send email with Check Boxes

How can I send an email with check boxes in it and get it returned to me
automatically when receipient has selected their preferred criteria. Email
will also have voting buttons
 
Use word and the forms toolbox for the checkbox. I wouldn't use voting
buttons but ask the people to reply to the mail...
