schema and modifications

where can I see a list of default attributes in a windows 2000 / 2003 server
AD, and where can I see what changes Exchange 2000 / 2003 makes to these
defaults?

The AD schema is documented (among other places) on MSDN. See this link:

The changes that Exchange makes are contained in LDIF files on the CD,
although I admit it isn't pretty to go through them. This might be a better
