How do I include a graphic logo in my return address for envelopes in Word
2003? I had no problem with Word97's "EnvelopeExtra1"

I don't know whether EnvelopeExtra1 and 2 still work in 2003 as long ago, I
created a template for my envelopes that is set up the way I want it so that
when I want an envelope, I just use File>New and select that template.

I would suggest that you do the same.

I use an envelope template also (there's one to download from my web site)
