REPORT FROM QUERY WITH COMBO BOX

Im trying to make a report from a query. I have a combo box that list several training example Fire Safety, Suicide, Redbook. I also have a date field. I want my report to have each one of these trainings as a title. I would like them in a column like this example below. Can this be done? Any help appreciate

visualize what you expect for results. Try providing a sample of your actual
data.

the items listed in my combo box i want as coloumn titles in my report. I dont know how to get the data in the

Duane what I am looking for in my report is the staffs name with all the training and dates of training he has had on the same row. I don't want his name listed 6 different row

You didn't provide any sample actual data, field or table names, or other
description of your data. Does a staff person take the same training more
than once? How would this be shown? There are several crosstab report
samples in the crosstab download at
