Reply option not working

I have created a customer form that two pages. Page one is the message and
on page two I have some check boxes, radio buttons, text fields. I am trying
to have the reply to the email and fill out page 2 of the form. I have the
option on the (Properties) tab to "Send form definition with item". When the
user tries to reply they get the message "The form you selected could not be
displayed", and can not reply. But if they click on forward they are able to
fill out the form, add a message to page one and everything appears to work.
I can not figure out why Reply does not work, nor does reply all.

You can set the form for the Reply action only to a form that's published in
Organizational Forms or in each recipient's Personal Forms library.
