reate a form in Outlook keeping the fields when sent

I would like to create a form in the body of an Outlook e-mail, send it to
recepients to fill out, and return to me.

Is there an easy way to do this to make it easy for the recipient to return
it filled out?

Office version?

It's not possible to get it to work at all if the recipients are outside your organization. If they're inside, then you need Exchange Server and permission to publish a form to the Organizational Forms library.

FYI, there is a newsgroup specifically for Outlook forms issues "down the hall" at microsoft.public.outlook.program_forms or, via web interface, at http://www.microsoft.com/office/com...spx?dg=microsoft.public.outlook.program_forms
