Radio buttons and/or checkboxes in WORD 2000

I'm not exactly new to Word, but I didn't see anywhere
else to post this question.
I would like to be able to (simply) insert either
checkboxes-(preferred), or radio buttons into a Word
document.
I've done some searching on the Web, but haven't really
come up with anything that I think would fill the bill.
If someone needs to know more specifically what the
application is to be, I would send you a sample document
if you respond via our e-mail address.

Clayton,

Since you are an experience Word user I will not go into
painstaking detail, but point you in the right direction.

Words Fax template (Contemporary for sure) has Style,
AutoText, and macros for checked and unchecked boxes. You
can use the organizer to copy these into your document
template.

Note: Checkboxes cannot be mutually exclusive. For that
feature you will need radio buttons.

If you just want the check boxes and not a protected form (and are willing
to double-click to check/uncheck the boxes), you may want to
use the kind of check boxes used in the fax cover sheet templates that ship
with Word. These consist of three components:

1. A MacroButton field that runs one of two macros depending on state.

2. A CheckIt macro and an UncheckIt macro.

3. Checked Box and Unchecked Box AutoText entries (these are just Wingdings
characters that have been saved as AutoText).

When the check box is empty (Unchecked Box is displayed), double-clicking on
the MacroButton runs the CheckIt macro, which substitutes the Checked Box,
and vice versa.

In order to use this type of box in your document, you need to Copy/Paste
the MacroButton field into your document/template, then use Organizer to
copy the macros and AutoText entries to your template (it has to be a
template because documents can't store AutoText entries, but you could copy
them to Normal.dot if you're just creating a document rather than a
template).
