Questionnaire ?

Would anyone be able to point me in the right direction of
learning how to set up a spreadsheet to serve as a
research questionnaire ? I was hoping to automate it such
that it could be sent out and when people answer questions
the results automatically tabulate on another sheet within
the workbook.
