questionnaire?

I am having problems sending a questionnaire form out and having people read
it. They can't see the checkboxes...
 
Have you put the checkboxes not only on the "Compose Page" but also on the
"Read Page"? In case you didn't, you should do so. Also go to the
"Properties" box in Design mode and make sure that "Send form definition
with item" is checked.
