Questionnaire

Help!! I'm a beginner with access and I should do a questionnaire. By
reading your conversations I understand that i should have 4 tables.
But i don't still understand how to create the question's table??? Am I
supposed to put a question in every field of this table??? Boh... This
is so difficult!!!
My questionnaire have 18 questions and for each question there are 5
answer choices (every person must choose a number from 1 to 5)
