Query form problem

I have created a query with a parameter input. It works fine run as a query,
but I have created a form from it. When I run the report it prompts for the
necessary user input, but does not restrict the report at all. Can someone
help me? Thanks in advance.!
 
You seem to change from form to report in the middle of your message. Can
you provide the SQL view of the report's record source and tell us what
happens when you open the report?
