Problems printing forms

Hello, we have an issue when printing forms, It is not displaying the form in the print preview or when it prints. We tried to clear the cache in outlook, also manually deleting the cache files in the Application Data/Microsoft/FORMS and still the same. What could be the problem here? Attached are screenshots on how it prints and how its supposed to look.
