Problems Connecting to an existing database

From the "Form Properties" dialog box, I have configured my form to connect
to a database, and I receive a successful connection, update, etc. message.
However, when the page is published and the user enters his/her information,
they receive the confirmation page when clicking submit, but the information
is not found in the database.

Please help!

I have tried several things to make sure my form was connected to the
database, but I can't see any information in the results of the form.

I had this problem once and it was because the web hoster did not have the web
identified as an "application root". Might be something to check with them.
