Problem with form - need help

I created a form for customers to submit data via email. Everytime you go to
submit the info on the form - by hitting the submit button - you get me logon
and password showing up on the screen. This only happens once - if you filled
the form out again and hit submit it doesn't show up again. Any clue to what
is happening?

if you are also writing to file, make sure the results file is in a folder w/ write permissions
(by default the _private folder)

thanks stefan - following the instruction of frontpage - when sending to
email only the the default file is removed so no file is there just the email.
