Pre-inserted text in subject line

Hi all

In order for external emails to be sent in our company, users have to begin
the subject line with particular text, for this example, we'll say that the
text is the users initials.
I have set up an email template with my initials pre-inserted in the subject
line, but this obviously can't be used when replying/forwarding to an email
or a meeting request. How can i perform this automically for email
replies/forwarding or meeting requests?

The ideal solution would be a COM add-in, installed on each user's machine, that responds to the events that occur whenever a user creates a new message.
