Populate form fields from query

I want to populate multiple text fields on a form from a
single drop down box. The drop down comes from a query. I
am able to get a single field to accept a value. FirstName
will will put the first name in the FirstName field, but
not any other fields, like company, last name, ...
Anybody know how to do this?
