Please help me create pop up questions

I need to create a questionnaire so that a question pops up along with
spot to answer the question. Once the question is answered, the nex
question pops up, etc....

Obviously, from there I would need the answers to populate
spreadsheet.

Help please!!
 
You might check out the VBA Help files on MsgBox and InputBox and see if
they spark any ideas.
