Page number help, please?!

I'm transfering a book that was typed on a typewriter to Word so it can be
sent out to the publisher, and I need to have the authors last name and the
page number in the upper right-hand corner of each page (ex. Karnes - 1;
Karnes - 2...). Have inserted the page numbers as requested but I can't seem
to figure out how to get the last name in there as well.

Please help! This is the one thing stopping me from submitting the book to
the publisher for review. Thank you so much!

You'll probably find it most effective to remove the page numbers you've
inserted (assuming you used Insert | Page Numbers) and insert the entire
phrase in the header. See
