outlook template qestion

I want to create a form/template that users (outlook & hotmail) can fill out
a couple fields and then send to a secretary. Can someone please tell me
where to start? Thanks

Where do you get stuck? Send them the email without the fields filled in and
ask them to press Reply to fill in the missing bits.

It doesn't matter. Plain Text should work already. For instance;
