Outlook form with .mdb connection

How do I set a form up (that I will publish to my company's
Organizational Form library) that connects to an Access database on
the server?

I don't want it to draw records from the database; when a person
clicks Send, I want the fields to populate the existing database.

In Sharepoint, you just link the sources and presto!, but Sharepoint
doesn't offer tabbed controls. Is there a similarily straightforward
way to link the form to the database in Outlook?

I have read and reread Sue Mosher's Outlook Programming book and it
doesn't help.
