Outlook Form Samples?

Hi All!

Doesn't anyone know a place where I can get a sample of a few forms in
Outlook? I am pretty confident I can do it if I have something to work from,
but I can't find any.

Many of the samples are for the Contacts and not quite what I am looking for.
