Outlook Custom Form not received

I'm in a test environment with two identical computer configurations
(Exchange Server). When I create and use a custom form, the recipient
computer shows the standard Outlook form. How do I correct this?
 
I saved the form in the Outlook Template folder and used the tools; forms;
choose forms to create the email. I was unaware that there was a read layout
associated with the process. Is there an additional step I missed (I'm in
Outlook 2003)? Is there a link to the process?
 
I'd say you missed at least one, maybe two steps:

1) Design a read layout by clicking the Edit Read Layout button in design
mode.

2) Either publish the form to the Organizational Forms library or check the
"Send form definition with item" box on the (Properties) page in design mode
before you save the form as an .oft file. (Don't do both!)
