outlook custom fields don't show up in mail merge

I have created several new forms in outlook with some special custom fields.
when I use word to try to create a mail merge form using these new custom
fields I cant seem to find the custom fields listed in the field chooser box.
Any ideas.

The fields must be defined in the folder, not just in the items/forms, and you must start the merge from Outlook, not Word.
