Outlook Address Form

I have a question. We have a long-standing email address "(e-mail address removed)"
sort of a deal. It's a service email address in Outlook. We want to
implement a Form to be filled out whenever a user intends to email us. Is it
possible to achieve this in outlook? - A form within the email based solely
upon selecting our email address in their global? I'm aware of InfoPath and
other fixes. But can we do this in Outlook?

Are the people mailing this address only your internal users or are they
external Internet users?

I ask because I want to understand how you'll guarantee that people mailing
'faqs@company' are actually using Outlook. It seems like the best solution
is to just create a web form that sends and email.
