outlook 2003 displays duplicate exchange mailbox

I recently upgraded to Outlook 2003. I have a single email account on an
Exchange 2003 server. The server was recently upgraded from 5.5 to 2003.

Outlook 2003 displays my mailbox twice. Once with an icon depicting folders
(similiar to my archive) and the other icon looks like a house with a
calendar behind it (home?)

How do I get rid of this duplication ... I want to know before deploying to
900 users.

Creating a new mail profile will remove the unwanted second account listing.

I have done that several times and your right they do not appear ... but
within a matter of minutes they re-appear. I can't figure out how to
permanently get rid of them. Is there a significance to the different icons
.... I can't find any reference as to what they imply.

The one with the little house next to it is your "Outlook Today" - your
mailbox to which items are delivered and which holds your defaault folders.
I'm afraid I can't explain the other. If you right-click and choose
Properties, do you get any clues to its identity?
 
Right clicking and displaying Properties displays exactly the same information.
Could the Folder Icon Mailbox be associated to the fact that at one time,
during out Exchange Upgrade, we had both the Exchange 5.5 Server and 2003
Server up simultaneously and somehow Outlook (even though I've re-installed)
is remembering that.

If I add another Exchange Mailbox that I have access to, it will show up
twice, with both the icons.

I've disabled "Cache Mode" thinking it may have something to do with an
offline mailbox but that's had no affect.

Perhaps, but that should have been cleared when you created the new mail
profile in Control Panel.
