Out of office assistance does not reply back to external emails

The out of office assisstant only sends a reply back to everyone in the
office, not to anyone outside the office. It seems like a useless tool if
none of my clients can recieve a reply saying I'm out of the office. Do I
have to set something else up in order for it to work?
