Opening form from button

I want folks to be able to open a form with a button, I have the form linked
to open only the ones that match the current site they are viewing, but what
I would like is for the form to open and allow a new entry only. The form is
set up so that when they then click the Print Report button it opens and
prints the report based on the form that is open.

Any help is always appreciated!
