Opening a form with default values

I'm opening a form from a switchboard in edit mode and cannot figur
out how to open with empty text boxes. Can anyone help?? I do hav
the default value set as blank

This might be explained better as opening a form to a "refreshed" o
"blank" state

If I need to explain myself better or you need more info, please le
