Notes; Unknown at the top of the page

I am working with outlook 2003. I have made a note and went to print it off
it shows "unknown" left side in bold title line. How do I remove this or
get it to show my name instead of "unknown"
 
Have you set a name for the account you use? Tools | Options | Mail
settings
