Good Morning All: I need to design a Form accessible to
all employees that has several fields, ie. First, Last,
Phone, etc. Once this form is filled out it will be
emailed to point person who will use this information to
fill out an access database. I've designed a message form,
however it will not let me choose it "When posting to this
folder use, use:" MYFORM. It says that "you cannot create
an item of this type in this folder". Basically what i
need is a custom Mail Message form so that it can be
composed and sent to a person. Any help or suggestion
would be appreciated.

Message forms are designed to be sent, not posted to a folder. Why are you trying to make it the default for a folder? See http://www.slipstick.com/dev/launchform.htm for various ways to launch a custom form.
