I did a google search on

Came up with this.

This references a bug related issue with a previous version. There is also a recommendation to upgrade the firmware of the drive as well as slow the burn process.

This one references the type of disc and drive. Do you have the proper discs for the drive?
I notice that this shows in the log: Sense Key: 0x03 (KEY_MEDIUM_ERROR)

Also, this:

Single Layer DVD+R? Burning a dual layer image?

Supports my theory that there is an issue with your disc somehow. Either it is a cheap disc, not the correct layering match with the drive, etc.
