Name inserted in reply

I'm trying to reply to an email which requires me to fill in a form. Every
time I type it inserts my name in brackets and I have to go back and delete
that. Since this is a checklist form with dozens of lines to fill out I want
to turn off the name insertion but I can't. I already tried
Tools>Options>Email Options but the box for Mark my comments with is greyed
out. What can I do to turn off this feature? Thanks.

I forgot to put in that this is Microsoft Outlook 2000 (version in about is
9.0.0.2711)
