Mysterious email sent from Outlook 2003 without To: info

A user of mine had an email sent out with an attachment without her notice.
I checked her "Sent Item" and "Deleted Item" and did not find that message.
I asked the recipient to send me a copy of the internet header of the message
and found NO "To:" specified in the header information. How could it be?

My questions:

1. Is it possible to send out an Outlook email without specifying "To:" and
"Cc: and "BCc"?
2. How is it possible there was no message in "sent Item and "Deleted Item"
folders (send a copy to sent item folder was configured and the deleted item
folder was not cleaned)
 
did a virus do it?

No. But is is possible, in general, to send SMTP mail without a To, Cc, or
any other header.
2. How is it possible there was no message in "sent Item and "Deleted
Item" folders (send a copy to sent item folder was configured and the
deleted item folder was not cleaned)

Outlook may not have sent it. Some malware contain its own SMTP client.
